How it works

Dosulepin is a tricyclic antidepressant that acts by increasing transmitter levels at central synapses, so producing a clinical antidepressant effect. In common with other tricyclics, Dosulepin inhibits the reuptake of noradrenaline and 5-hydroxytryptamine, with a significantly greater action on the reuptake of noradrenaline.

Bone marrow depression, agranulocytosis , Hypersensitivity reactions , Inappropriate antidiuretic hormone (ADH) secretion, endocrine side effects , Hyponatremia , mania and paranoid delusions, suicidal ideation and suicidal behavior ,Tremors, drowsiness, convulsions, movement disorders, dizziness and headache , hypotension and cardiac arrhythmias , Dyspepsia, Abdominal pain, diarrhea and nausea , Skin rashes, sweating .

Drug Interactions

Selective serotonin reuptake inhibitors (SSRI) , alcohol and narcotic analgesics , sotalol, terfenadine, astemizole, halofantrine , bethanidine, debrisoquine, guanethidine , Barbiturates , methylphenidate.

Indication

Dosulepin is indicated in the treatment of symptoms of depressive illness, especially where an anti-anxiety effect is required.

When not to Use

Dosulepin is contraindicated in patients with recent myocardial infarction, any degree of heart block or other cardiac arrhythmias. Dosulepin is contraindicated for the treatment of mania. Dosulepin is contraindicated for patients with severe liver disease. Dosulepin is contraindicated for patients with known hypersensitivity to Dosulepin hydrochloride or any the excipients.


Precaution

Dosulepin is associated with high mortality in overdose. There is a low margin of safety between the (maximum) therapeutic dose and potentially fatal doses.


Warning 1

Depression is associated with an increased risk of suicidal thoughts, self harm and suicide (suicide-related events). This risk persists until significant remission occurs. As improvement may not occur during the first few weeks or more of treatment, patients should be closely monitored until such improvement occurs .

Warning 2

Avoid use in patients with a history of epilepsy, thyroid disease, mania or urinary retention and in those with narrow-angle glaucoma or symptoms suggestive of prostatic hypertrophy. Patients with hepatic impairment and those undergoing electroconvulsive therapy should also avoid dosulepin use .

Warning 3

Dosulepin may increase the risk of cardiovascular toxicity (cardiac arrhythmias, conduction disorders, cardiac failure and circulatory collapse), especially in the elderly.
